What is RSCM?

RSCM is the process of assigning specific health risk statuses to patients by a variety of matrices. Think of it as each patient having his or her own level which quickly identifies that patient’s health status.

Level 1 patients are healthy with no medical issues, while Level 6 patients are in the “catastrophic extreme” range.

The main goal of RSCM is to identify and help patients achieve the best possible health and quality of life by controlling, if not preventing, chronic disease, keeping patients from moving into higher-risk categories, and to keep costs stabilized or reduced in the process.
