CPC is a new four year CMS initiative available to primary care practices located in the Capital District / Hudson Valley (NY), the Cincinnati / Dayton region (OH), the Greater Tulsa region (OK), and all of NJ, AR, CO and OR. Additional states or regions may be added over time.
The purpose of the initiative is to test a healthcare service delivery model. As an incentive for participation in the CPC initiative, practices will receive a monthly care management fee averaging $20 per Medicare Fee-for-Service beneficiary for the first two years of the program and $15 for the third and fourth years.
